The Approbation is the state license to practice independently as a dentist. Without it, you can only work as an assistant under supervision. Each German state grants it separately, in Hamburg through Behörde für Wissenschaft, Forschung, Gleichstellung und Bezirke.
Who can apply for Approbation
Basic requirements to check before any expenses.
✅ Dental degree: at least 5 years from an accredited or EU-recognized university.
✅ Right to live in Germany: any status (Aufenthaltstitel, Niederlassungserlaubnis, Blaue Karte EU, Schutzstatus) that allows employment.
✅ German proficiency B2 + C1 (Fachsprache): B2 general (Goethe, TELC, ÖSD), C1 medical (Patientenkommunikationstest or Fachsprachprüfung).
✅ Reliability (Zuverlässigkeit): no criminal record threatening patient safety. Verified via Führungszeugnis.
✅ Health fitness: medical certificate confirming professional fitness, hepatitis B/C and HIV test.

Submit to Approbationsbehörde

Equivalency assessment (Gleichwertigkeitsprüfung)
Determines if your education matches the German one. In 80% of non-EU diplomas, «significant differences» are found. You then receive an official decision. Kenntnisprüfung is required.
Berufserlaubnis (temporary license, optional)
Temporary license for up to 2 years while preparing for the Kenntnisprüfung. Allows work under supervision of a licensed colleague. Applied at the same office. Not all states grant it. in Hamburg under specific conditions.
Fachsprachprüfung
C1-level medical German exam. In Hamburg via the relevant Kammer. Includes: anamnesis with a patient, writing a doctor's letter, writing an insurance report. Cost 350-500 €. Preparation 3-6 months.
Kenntnisprüfung
Oral exam on dental knowledge. Includes a theoretical and practical part (patient examination, treatment plan). Cost 800-1200 €. Often passed on the second attempt, first-attempt pass rate is about 50%. Up to 3 attempts allowed.
Approbation granted
After passing the Kenntnisprüfung: final document check, issuance of the Approbationsurkunde, registration with Zahnärztekammer Hamburg. Cost 200-400 €. You can now open your own practice or work independently.

Waiting for the complete document folder before applying
«I'll wait until I have all the documents.» Don't wait. Submit with what you have. The office will tell you what's missing and you'll add it. Every month of delay equals a month of delay to Approbation.
Getting translations done back home
Translations from Ukraine or other countries aren't recognized. You need a German sworn translator (Beeidigter Übersetzer). Find one via the BDÜ (German federation of translators). Saving here costs time later.
Preparing for Kenntnisprüfung alone
The Kenntnisprüfung scope is broad: restorative, prosthetics, surgery, periodontology, pediatric, orthodontics, radiology, pharmacology. Solo prep takes 1-2 years. With a course, 6-9 months. The 1500-3000 € investment pays off in time.
Taking the Kenntnisprüfung without hospitation
Hospitation, unpaid observation in a German practice 3-12 months. Gives you German clinical routine, terminology, documentation. Without it, passing the Kenntnisprüfung is much harder. Find one via the Kammer or personal contacts.
Working illegally without Approbation
Practicing dentistry without a license is a criminal offense (§ 132a StGB). Up to 1 year in prison or fine up to 250 000 €. Don't risk it. Work as an assistant (ZFA) officially until you receive the license.
