Klaus came in with a toothache and left with a treatment plan for 2400 euros. Implant plus crown, a standard situation for a 55-year-old. His GKV covered 480 euros. Klaus paid the remaining 1920 euros himself. «Why did nobody ever explain this insurance to me?» he asked on his way out. I stayed silent. Because for him the question was already irrelevant. Insurance works prospectively, not retroactively.

This is the most important financial advice I give my patients, especially the young ones. Zahnzusatzversicherung is the most worthwhile health investment in Germany. 15-25 euros a month at 25 = thousands saved at 45-55. I signed up at 24 myself, my son will get a policy as soon as he is on German insurance.

Zahnzusatzversicherung is a private supplementary insurance on top of statutory health insurance (GKV) that bridges the gap between what GKV pays and actual dental costs.


What does GKV pay. and what is missing?

Statutory insurance covers Regelversorgung (standard treatment). For dental prosthetics, GKV pays a Festzuschuss of 60% of the standard treatment cost. With a documented Bonusheft (annual check-up visits), the subsidy rises to 70% or 75% after 10 years.

But Regelversorgung is not always what you want. Metal-ceramic crown instead of full ceramic. Removable denture instead of implants. You pay the difference. And professional dental cleaning (PZR) is not a GKV benefit. it costs €60–120 per session.


How Zahnzusatzversicherung works

Supplementary dental insurance reimburses part or all of the costs GKV does not cover. Good tariffs provide 90 percent reimbursement for crowns, bridges, implants, inlays.

Key parameters when comparing

Leistungsbegrenzung (payout limits): Most policies cap payouts in the first 3-5 years. For example, max 500-1500 euros in year one, then unlimited. If you plan major work soon, choose a tariff with higher limits or «Sofortleistung».

Wartezeit (waiting period): Typically 3-8 months after policy start. PZR is often covered, treatments are not yet. Tariffs without a waiting period cost about 30 percent more.

Pre-existing treatment (Vorerkrankung): Treatment your dentist recommended BEFORE signing is excluded. Missing teeth or already-diagnosed cavities are excluded too.


Top tariffs 2026 by Stiftung Warentest

Stiftung Warentest tests tariffs annually. Top 5 from the 2025-2026 ranking:

  1. Allianz MeinZahnschutz 90/90 AR. 90 percent dental prosthetics, 90 percent PZR. No limits after 5 years. 20-35 euros/month for ages 30-40. The most popular tariff among dentists themselves.
  2. HUK24 ZZ Pro90. 90 percent coverage, cheaper than Allianz but weaker on adult orthodontics. 15-25 euros/month. Good value.
  3. ERGO Direkt Zahnersatz Sofort. No waiting period, useful if you plan treatment soon. 25-40 euros/month.
  4. DFV ZahnSchutz Exklusiv 100. 100 percent coverage, the most expensive. 35-55 euros/month. Worth it with active plans for orthodontics or implants.
  5. BARMER Zahn90. Through your statutory health fund. Bonus: integration with the main GKV card. 18-30 euros/month.

I personally recommend Allianz or HUK24 for most patients. Do not sign up through comparison portals (Check24, Verivox), they take a commission from your premium for life. Sign up directly on the insurer's website.

What a good insurance costs

Quality tariffs for adults 30-40: 15-30 euros per month (180-360 euros per year).

Compare with one uncovered crown: 600-1200 euros out of pocket. A single treatment already pays back several years of premium.

For young patients (under 25), premiums start at 8-15 euros per month.


When to sign up and what mistakes to avoid

Ideal moment: before 30, while healthy and with no diagnoses. The premium gets locked in low for life.

Still okay: ages 30-45 with no active treatment recommendation. Premium higher, but still worth it.

Common mistake: waiting until the dentist says «you need an implant». By then it is too late, the Heil- und Kostenplan excludes that treatment.

Strategy: buy the latest Finanztest online edition on test.de for 7-10 euros. Detailed comparison of all tariffs. Apply directly with 2-3 providers. Premiums can differ by 30 percent for the same coverage.
