Imagine this: you brush twice a day, floss, use mouthwash. and your dentist still says "you have tartar buildup." Or worse: "you have early-stage gum inflammation." How?

The answer is simple: an ordinary toothbrush physically cannot reach where a dentist can with professional instruments. That is the essence of PZR. Professionelle Zahnreinigung (professional dental cleaning). In Germany it's advertised everywhere, and many patients wonder: is it really necessary, what exactly are you paying 100+ euros for, and is it worth it?

Let's look at this honestly, with facts.

What happens during PZR: step by step

PZR is not just "more thorough teeth brushing." It is a structured procedure with several stages:

1

Plaque disclosure

A special dye stains all areas where bacteria have accumulated. You literally see in the mirror exactly where your home brushing falls short.

2

Tartar removal

An ultrasonic device removes mineralised deposits. both above and below the gumline. No toothbrush can do this: tartar is as hard as bone.

3

Airflow

A pressurised water-powder jet removes soft plaque and pigment stains from coffee, tea, red wine, or cigarettes. After this step, teeth noticeably brighten.

4

Interdental cleaning

Special floss, interdental brushes, and curettes clean the spaces between teeth.

5

Polishing

Tooth surfaces are polished smooth. Smooth teeth are harder for bacteria to colonise. new plaque forms more slowly.

6

Fluoride treatment

A final application of fluoride varnish strengthens enamel and reduces sensitivity.

The entire procedure takes 45 minutes to an hour and a half.

How often is PZR needed?

The standard recommendation: once or twice a year. For high-risk patients. smokers, diabetics, periodontitis patients, those wearing braces. three to four times per year.

PZR cost in Germany

PZR is not included in the standard GKV benefits catalogue. The realistic price range in 2025โ€“2026:

๐Ÿ’ถ 80โ€“120 โ‚ฌ. average in most cities

๐Ÿ’ถ 120โ€“150 โ‚ฌ. practices in major cities (Hamburg, Munich, Frankfurt)

๐Ÿ’ถ 150 โ‚ฌ and above. specialist periodontology centres

Before the procedure, you have the right to request a Kostenvoranschlag (cost estimate).

Dental cleaning insurance Germany: what does GKV pay?

GKV covers only the annual tartar removal (Zahnsteinentfernung). a short procedure without airflow, polishing, or fluoride treatment. The full PZR is a Selbstzahlerleistung (self-pay service).

However, many health insurers offer voluntary supplementary benefits (Satzungsleistungen):

๐Ÿฅ KKH. up to 120 โ‚ฌ per year (up to 60 โ‚ฌ per session, 2x annually)

๐Ÿฅ AOK Niedersachsen. up to 500 โ‚ฌ per year

๐Ÿฅ AOK Bayern. up to 100 โ‚ฌ per year (50 โ‚ฌ per session, from age 15)

๐Ÿฅ hkk. one free PZR per year at DentNet partner practices

๐Ÿฅ IKK Classic, DAK, BIG Direkt. partial reimbursements of 20โ€“70 โ‚ฌ

Techniker Krankenkasse (TK) and the largest AOK funds in many regions do not reimburse PZR. Read more about navigating the German dental system in our article on dental anxiety in Germany.

Private insurance (PKV): a different story

If you have private health insurance (PKV), most plans cover PZR fully or substantially. 1โ€“2 times per year. With GKV, consider a Zahnzusatzversicherung (supplemental dental insurance) from 7โ€“15 โ‚ฌ per month.

Is it really worth it?

The scientific evidence is mixed. There is insufficient proof that PZR protects against cavities better than home hygiene alone. What is clearly established: PZR is essential before and after periodontitis treatment, and for smokers, diabetics, and immunocompromised patients.

For everyone else. one PZR per year is a reasonable investment in prevention and early diagnosis.

Do you get regular PZR cleanings. or do you stick to what your Krankenkasse pays for? ๐Ÿ‘‡
