Maria avoided close contact for 5 years. «My husband hinted at it, friends would step back in conversation. I had my stomach, liver, even gastroscopy checked. All fine. Doctors said 'all normal, maybe psychosomatic'.» At the dental exam we found the cause in 5 minutes: deep gum pockets at the back molars (PSI 3-4), tongue with a thick white coating, two old fillings with hidden caries underneath. After 4 months of combined treatment, the smell was gone for good.
Halitosis (from Latin halitus, «breath») is persistent bad breath. Affects 25-30% of adults (Cochrane 2024). 50% don't know about their own breath (adaptation), 30% know but are too embarrassed to discuss, 20% actively look for solutions.
Where the smell comes from: 90% of causes are in the mouth
Common myth: «it's from the stomach.» In fact, 90% of cases originate in the oral cavity. Bacteria break down proteins and release volatile sulfur compounds (VSC), which smell like rotten eggs or aged cheese.
👅 Tongue (60% of cases): the back third of the tongue accumulates bacterial coating. Whitish-yellow film, main VSC source.
🩸 Periodontitis (20% of cases): deep gum pockets host anaerobic bacteria. Characteristic «rotten, earthy» smell.
🦷 Caries and old fillings (10% of cases): bacteria grow under leaking fillings. Patient often unaware of hidden decay.
💧 Dry mouth (xerostomia, 5% of cases): reduced salivation from medications (antidepressants, antihistamines), age, mouth breathing. Saliva is the natural antibacterial.
🦴 Tonsillitis and tonsil stones (3-4% of cases): «stones» in tonsils made of accumulated protein, dislodge during swallowing, smell distinctively.
🏥 Systemic causes (1-2% of cases): GI (gastritis, reflux), kidneys (uremia), diabetes (acetone smell), liver. No more than 10% of all cases despite the popular myth.
How to know if you have bad breath
Paradox: 50% of people with halitosis don't know about it. Olfactory adaptation to your own smell.
Spoon test
Use a plastic spoon or tongue scraper to take a sample from the back third of your tongue. Wait 30 seconds, smell it. Strong smell means halitosis. Fastest home test.
Wrist test
Lick your wrist, wait 10 seconds, smell it. Most sensitive self-test. Strong smell means see a dentist.
Ask someone close
Most reliable. Talk to your partner or family member about anything and ask honestly: «I'm curious, is my breath fresh?» Close ones are adapted and won't volunteer the info, but they'll give an honest answer when asked.
Halimeter (professional device)
Measures VSC concentration in mouth air. Norm under 100 ppb, halitosis 150-1000+ ppb. Available at specialized halitosis clinics. Price 30-60 € per test.
5-step elimination plan
Works in 90% of cases. Order matters.
Daily tongue cleaning
Tongue scraper (Zungenreiniger) 1-2 € at any pharmacy. Morning and evening before brushing. Scrape the back third 3-5 times until the tongue looks clean. Reduces VSC by 60-80%. Fastest and cheapest step.
Daily flossing
Food and bacteria between teeth, where the brush can't reach. Once a day in the evening. Especially with gaps or crowns. Reduces VSC by 20-30%.
Professional cleaning (PZR)
Every 6-12 months. Removes tartar and plaque from areas inaccessible to brushing. Includes ultrasound, polishing, fluoride. 80-120 € private. Especially crucial with periodontitis.
Treat periodontitis and caries
If PSI 3-4, PAR therapy needed (GKV-covered after Bonusheft). Replace caries and failing fillings. Without this the smell won't go away. the source remains.
Boost salivation
Drink 2-3 liters of water daily. Chew sugar-free gum 10-15 min after meals. For severe xerostomia, artificial saliva (Glandosane, BioXtra, 10-15 € at the pharmacy). Cut caffeine and alcohol, both dry the mouth.
What doesn't work (common myths)
❌ Flavored chewing gum: masks for 10-15 min, doesn't treat the cause. Sugar-free gum with xylitol is better (stimulates saliva).
❌ Breath spray: same. 5-10 min of effect.
❌ Alcohol-based antiseptic rinses: temporarily reduce bacteria but alcohol dries the mouth, long-term can worsen the smell. Better: alcohol-free 0.12% chlorhexidine for short 2-week courses.
❌ «Treating the stomach» with antacids: only if you actually have reflux or gastritis. In 90% of halitosis the stomach isn't the cause.
❌ Home remedies (mint, parsley, activated charcoal): mild masking for 5-10 min. Don't cure.
When to see a halitosis specialist
If self-care doesn't help within 4-6 weeks, see a halitosis specialist. That's a dentist with extra qualification. Hamburg has several clinics.
🏥 Specialized halitosis consultation: full oral exam, VSC measurement via Halimeter and Oral Chroma (exact sulfur compound ID), tongue swab for anaerobes, salivary flow analysis. Cost: 150-300 €.
🩺 If dental cause ruled out: referral to gastroenterology (GI), ENT (sinusitis, tonsils), endocrinology (diabetes), GP (kidneys, liver).
📊 Hamburg halitosis points of contact: UKE Department of Oral and Maxillofacial Diseases, specialized private clinics. Book directly, GKV doesn't refer for halitosis.
