Irina, 74, came in with a new full denture made at another clinic. She couldn't speak, the denture kept falling out, her gums hurt. I looked: old-style acrylic, no implants. She was on 5 medications (blood pressure, heart, antidepressant, diuretic, antihistamine). Dry mouth 9/10. I explained: «Irina, the problem isn't the denture. You have xerostomia from polypharmacy and 50% alveolar atrophy after 5 years without teeth. An overdenture on 2 locators for 5500 € would fix both». Three months after implants and a new denture, she was eating steak again. I see this kind of situation with 70+ patients every week.

Old age brings unique dental challenges: drug-induced xerostomia, bone loss, candida under the denture, progressive periodontitis, changes in enamel colour and translucency. Among 65+ patients in Germany, 32% wear full dentures, 45% partials, 12% have implants (DMS 6 2023). In this article: how I treat elderly patients, saliva care, denture relining, GKV coverage, and when an implant is still possible.


Dry mouth: when medications steal saliva

Xerostomia. the medical term for dry mouth. has become a genuine epidemic among older people. The cause: medications. According to medical literature, over 400 drugs have dry mouth as a side effect.

The most dangerous groups: urological drugs increase xerostomia risk nearly 6-fold (OR = 5.91), antidepressants nearly 5-fold (OR = 4.74), antipsychotics 2.6-fold. Plus diuretics, blood pressure medications, antihistamines.

A person in their 70s taking 5,7 medications (Polypharmazie) practically guaranteed has reduced saliva flow. even if they don't notice it clearly.

Why this is critical for denture wearers

Saliva is not just moisture. It's antibacterial, protects the mucosa, and acts as "lubricant" for the denture. With xerostomia: the denture fits poorly, caries risk rises sharply, the mucosa under the denture becomes inflamed (Prothesenstomatitis).


Removable denture care rules

Daily

Remove the denture at night. the gums need rest. Clean with a special brush under running water. without toothpaste (toothpaste is abrasive and scratches acrylic). Use effervescent tablets (Prothesenreiniger, e.g. Corega Tabs). kills bacteria and Candida fungus.

Every 5,7 years

The denture needs replacing. Gums and bone change. A denture made 10 years ago no longer fits properly. even if it "still holds".

Implant-supported denture. modern alternative

For elderly patients with total tooth loss in the lower jaw, a well-studied solution: Stegprothese on 2,4 implants. The patient removes it for cleaning, but during the day it is firmly anchored. Cost: €10,000,18,000 per jaw. GKV only covers the prosthetic construction, not the implants.

5 problems I treat in elderly patients every week

Concrete cases from 16 years of practice.

1. Candida under the denture (denture stomatitis)

Red, inflamed mucosa under the denture, burning, discomfort. Caused by Candida albicans. Most common in patients who don't take their denture out at night. Treatment: Nystatin cream for 2 weeks + denture cleaning with chlorhexidine + take the denture out at night, no exceptions.

2. Root caries

Gums recede with age, exposing root surfaces that have no enamel. Caries develops 3-5 times faster on root cement. I use Duraphat fluoride varnish 4 times a year plus 5000 ppm prescription paste.

3. Polypharmacy and drug interactions

Anticoagulants (Marcumar, Eliquis) need cardiologist coordination before surgery. Bisphosphonates (for osteoporosis) carry a high risk of ONJ (osteonecrosis of the jaw) after extraction. I always ask about medications before surgical procedures.

4. Cognitive impairment

With dementia or Alzheimer's, patients forget to brush teeth and dentures. Caregivers must clean the denture and gums twice a day. I work with Pflegedienste in Hamburg and provide written instructions.

5. Alveolar ridge atrophy

5-10 years of edentulism → 60% bone loss. The denture holds poorly. Solution: 2 implants in the lower jaw (Locator overdenture), radically stabilising. Total 4000-6500 €.
