Caries develops slowly. From the first white spot to tooth pain takes 1-4 years. During that time the patient doesn't know about the problem, but the dentist can find it. The earlier it's caught, the simpler and cheaper the treatment.

Visual exam + ICDAS
The dentist examines each tooth with mirror and magnifying loupe. Uses ICDAS (International Caries Detection and Assessment System) scoring: 0 healthy, 6 visible cavity. Accuracy: 60-70% for open surfaces. Weakness: can't see between teeth or under fillings. Cost: included in GKV exam.
Bitewing X-ray
2-4 bite X-rays. Shows caries between teeth and under fillings. Most important method for adults. Accuracy: 80-90%. Dose: 0.005 mSv per image (= 2 hours of flight). Frequency: low risk every 2-3 years, high risk annually. Cost: GKV-covered.
Laser fluorescence (DIAGNOdent, KaVo)
Laser shines on the tooth, calcified caries fluoresces. Device shows reading 0-99 (0 healthy, 30+ suspicious, 50+ caries). Accuracy: 85-95% for occlusal caries (chewing surfaces). Weakness: false positives from tartar, fluoride. Cost: 10-30 β¬ private per tooth, not GKV.
Caries Detector (dye)
Special dye applied to the tooth during filling prep. Infected dentin stains, healthy doesn't. Helps remove only the affected tissue without overcutting healthy. Accuracy: high with correct application. Cost: included in filling price.
Transillumination (FOTI, Diagnocam)
Intense light shines through the tooth. Healthy tissue transmits light, caries appears dark. Radiation-free alternative for interproximal monitoring. Accuracy: 70-80%. Weakness: not for deep caries. Cost: 5-15 β¬ private per exam, not GKV.
What works best for each situation
π¨ Low-risk adult: exam + Bitewing every 2-3 years. Cheapest and effective. GKV-covered.
π© High-risk adult (many fillings, periodontitis, diabetes): exam + Bitewing annually + DIAGNOdent. Early detection critical.
πΆ Child 6-12: exam + DIAGNOdent (radiation-free when possible). Bitewing every 2-3 years only if at risk.
