Yuri hadn't seen a dentist in 6 years. Β«My teeth don't hurt.Β» On exam, 3 small interproximal caries found via Bitewing X-ray. If he'd waited until pain, he'd need endodontics (350-800 € per tooth) instead of a simple filling (80-180 €). 70% of interproximal caries is invisible without X-ray.

Caries develops slowly. From the first white spot to tooth pain takes 1-4 years. During that time the patient doesn't know about the problem, but the dentist can find it. The earlier it's caught, the simpler and cheaper the treatment.

5 diagnostic methods

1

Visual exam + ICDAS

The dentist examines each tooth with mirror and magnifying loupe. Uses ICDAS (International Caries Detection and Assessment System) scoring: 0 healthy, 6 visible cavity. Accuracy: 60-70% for open surfaces. Weakness: can't see between teeth or under fillings. Cost: included in GKV exam.

2

Bitewing X-ray

2-4 bite X-rays. Shows caries between teeth and under fillings. Most important method for adults. Accuracy: 80-90%. Dose: 0.005 mSv per image (= 2 hours of flight). Frequency: low risk every 2-3 years, high risk annually. Cost: GKV-covered.

3

Laser fluorescence (DIAGNOdent, KaVo)

Laser shines on the tooth, calcified caries fluoresces. Device shows reading 0-99 (0 healthy, 30+ suspicious, 50+ caries). Accuracy: 85-95% for occlusal caries (chewing surfaces). Weakness: false positives from tartar, fluoride. Cost: 10-30 € private per tooth, not GKV.

4

Caries Detector (dye)

Special dye applied to the tooth during filling prep. Infected dentin stains, healthy doesn't. Helps remove only the affected tissue without overcutting healthy. Accuracy: high with correct application. Cost: included in filling price.

5

Transillumination (FOTI, Diagnocam)

Intense light shines through the tooth. Healthy tissue transmits light, caries appears dark. Radiation-free alternative for interproximal monitoring. Accuracy: 70-80%. Weakness: not for deep caries. Cost: 5-15 € private per exam, not GKV.

What works best for each situation

πŸ‘¨ Low-risk adult: exam + Bitewing every 2-3 years. Cheapest and effective. GKV-covered.

πŸ‘© High-risk adult (many fillings, periodontitis, diabetes): exam + Bitewing annually + DIAGNOdent. Early detection critical.

πŸ‘Ά Child 6-12: exam + DIAGNOdent (radiation-free when possible). Bitewing every 2-3 years only if at risk.

🀰 Pregnant: visual exam + DIAGNOdent. Avoid Bitewing (possible after birth). Caries Detector safe.

πŸ‘΅ Over 65 patient: exam + Bitewing annually. Root caries on receded roots is common in this age, Bitewing sees it.

πŸ›‘ Suspected hidden caries: combo DIAGNOdent + transillumination + Bitewing. Highest accuracy 95%.

Why GKV doesn't cover newer methods

GKV covers standard diagnostics: exam, Bitewing, OPG. Everything else is private.

πŸ’° DIAGNOdent: 10-30 € private. Better than exam, but GKV deems standard exam sufficient.

πŸ’° 3D scan (CBCT/DVT): 200-400 € private. Only for suspected complex caries, cysts, or before implantation. GKV covers in specific cases.

πŸ’° Caries Detector: included in treatment cost. Indirectly GKV-covered.

πŸ’° Transillumination: 5-15 €. Radiation-free alternative for children and pregnant women. Private.

πŸ›‘ Zahnzusatzversicherung: covers 70-90% of private diagnostics. With insurance you can use modern methods at no out-of-pocket cost.
