Anna spent 6 months using charcoal toothpaste before her wedding. Β«It got worse: yellowing stayed and the enamel became sensitive.Β» At the clinic we did in-office bleaching in 1 hour, teeth lightened by 5 shades. Result lasted 18 months and counting.
Teeth darken with age for two reasons: 1) extrinsic staining from coffee, tea, red wine, tobacco (accumulates on enamel surface), 2) intrinsic staining from dentin aging, medications (tetracycline in childhood), trauma. Whitening restores the natural shade but doesn't change the color of fillings, crowns, or veneers.
5 methods: cost, efficacy, safety
In-office bleaching (Power Bleaching)
Dentist applies 25-40% hydrogen peroxide gel, activates with laser or LED. Duration: 60-90 minutes, 1 visit. Lightening: 4-8 shades per session. Result duration: 1-3 years depending on habits. Cost: 350-700 β¬ (private). Safety: safest, dentist controls, gums protected.
Home tray bleaching (dentist-supplied)
Dentist makes custom trays, provides 10-16% carbamide peroxide gel. Wear 4-6 hours daily or overnight for 2-4 weeks. Lightening: 3-7 shades. Cost: 250-400 β¬ (tray + gel course). Safety: high, weaker gel, lower sensitivity risk.
Drugstore strips (Crest Whitestrips, Blanx)
Pre-made strips with 6-10% peroxide. No custom tray. Lightening: 1-3 shades. Cost: 30-80 β¬ for a 14-day course. Risks: gum irritation (strip doesn't fit precisely), uneven lightening, tooth sensitivity.
Whitening toothpaste
Contains abrasives (RDA 100-200) or low peroxide concentrations. Lightening: 0-1 shade (surface stains only). Cost: 5-15 β¬ per tube. Risks: enamel wear with high RDA, sensitivity after 2-3 months. Don't use more than 2 weeks in a row.
Activated charcoal (TikTok trend)
Abrasive powder. Lightening: minimal and temporary. Cost: 5-10 β¬. Risks: heavy enamel wear (RDA 200-400+!), tooth surface becomes porous, darkens further within 6-12 months. NOT recommended. German Dental Society (DGZMK) officially warns.
When NOT to whiten
β Pregnancy and breastfeeding: safety data missing. Wait until after breastfeeding.
β Under 16 years: enamel matures until 16-18, bleaching interferes with maturation.
β Untreated caries or deep fillings: gel will reach pulp through microcracks, pulpitis risk.
β Thin enamel (amelogenesis, fluorosis, recession): can amplify sensitivity and damage dentin.
β Fillings, crowns, veneers on front teeth: they won't change color. Result: the filling will look darker than the whitened tooth.
β Peroxide allergy: rare but possible. Test small area first.
How to maintain the result
Without care whitening lasts 1 year. With proper care 2-4 years.
β First 48 hours after whitening: no dark drinks (coffee, tea, wine), no dark sauces (soy, berries, tomato), no tobacco. Enamel is open, dyes penetrate.
π₯ First week: white diet, rice, fish, potato, apple, banana, milk.
π¦· Brushing: soft brush, whitening toothpaste once a week, no more.
β Coffee and tea: drink through straw, rinse with water after.
π Smoking: any form (cigarette, IQOS, hookah) brings back darkness in 6-12 months.
π¦· Touch-up sessions: home tray with gel every 6-12 months.
